U.S. Defense Secretary Chuck Hagel said on Wednesday he has ordered two department-wide security reviews, after a deadly shooting this week in the Navy Yard in Washington.

During a Pentagon press briefing, Hagel admitted "something went wrong" with security check at military facilities. "We will find those gaps, as I said, and we will fix those gaps," he said.

Hagel said he has ordered reviews to examine physical security and access procedures for all Department of Defense (DOD) installations worldwide, and for granting and renewing security clearances, adding that the two reviews will be led by Deputy Secretary of Defense Ashton Carter.

In the meantime, Hagel said, an independent panel will conduct its own assessment of security at DOD facilities and security clearance procedures and practices.

The reviews came as questions grew as to how Aaron Alexis, a former Navy reservist with a history of mental health problems, was able to get a security clearance to enter the Navy Yard. Alexis, a 34-year-old information technology employee at a defense- related computer company, used a valid pass Monday to get into the Navy Yard and then killed 12 people before police shot him dead.

President Barack Obama on Tuesday ordered a review of standards for contractors and employees in the federal government in the wake of the shooting rampage.
